How to Make a Workers’ Compensation Claim

1. Report the injury to your employer

Ideally, you should notify your employer as soon as possible, ideally on the same day. This can be verbal, but a written record is strongly recommended.

2. See a doctor and obtain a First Certificate of Capacity

See a doctor of your choice and request a First Certificate of Capacity, which will provide details of the injury, diagnosis, work capacity and recommended treatment. This certificate is essential for the claim to proceed.

3. Complete a Workers’ Compensation Claim Form

Fill out a Workers’ Compensation Claim Form. The completed Workers’ Compensation Claim Form and First Certificate of Capacity must be given to the employer.  You should keep a copy of these documents for your records and take note of when you gave these documents to your employer and how (i.e. by email, by post, in person etc.)
